Incident Reports

U.S. based airlines are required to submit safety reports called Service Difficulty Reports (SDRs) for malfunctions involving aircraft systems in accordance with U.S. law. The Federal Aviation Administration (FAA) is required by Department of Transportation policy to investigate these incidents to determine root causes and to ensure corrective action is taken by aircraft manufacturers and airlines to fix the problem(s). These reports are crucial for maintaining and improving aviation safety, as they help to identify potential hazards, evaluate risks, and implement corrective measures to prevent future tragedies.

The foundation believes it is important to highlight select reports involving 737 MAX airplanes. We find the reports listed below concerning because of the history of the MAX and the repetitive and/or serious nature of the reports.
