Incident Reports

U.S. based airlines are required to submit safety reports called Service Difficulty Reports (SDRs) for malfunctions involving aircraft systems in accordance with U.S. law. The Federal Aviation Administration (FAA) is required by Department of Transportation policy to investigate these incidents to determine root causes and to ensure corrective action is taken by aircraft manufacturers and airlines to fix the problem(s). These reports are crucial for maintaining and improving aviation safety, as they help to identify potential hazards, evaluate risks, and implement corrective measures to prevent future tragedies.

The foundation believes it is important to highlight select reports involving 737 MAX airplanes. We find the reports listed below concerning because of the history of the MAX and the repetitive and/or serious nature of the reports.

CDANCIK 56558 08/15/2023OPERATED THE LEADING EDGE FLAPS AND VISUALLY EXAMINED THE ADJUSTMENT OF THE #8 SLAT. THE SLAT ADJUSTMENT IN ALL 3 POSITIONS (UP, INTERMEDIATE, AND FULL DOWN). CONFIRMED NORMAL OPERATION THROUGHOUT FULL TRAVEL. FULL RETRACTION WAS NORMAL, SLAT POSITIVELY ENGAGED THE DETENTS IN INTERMEDIATE, AND FULL CONTACT WAS MADE WITH THE DOWNSTOPS FULL EXTENDED (NO ROCKING) #8 SLAT OPERATION AND ADJUSTMENT WAS MATCHED WITH AND COMPARABLE TO THE #1 SLAT. AN EXTENDED SLAT WOULD HAVE CAUSED ADVERSE FLIGHT CHARACTERISTICS. NO ADJUSTMENTS REQUIRED REF MM 27-61-00-820-803 AND 27-81-00-860-801. OPS CKS NORMAL FOLLWOING INDICATION REPAIRS ON TSR714829.
